Home Health

The Home Health team works with physicians to bring health care to you in the comfort of your own home. Services offered to Home Health patients include: physical assessments, monitoring and education of medications, diabetes management, rehabilitation therapy, wound and ostomy care, post hospital discharge assessments, nutritional counseling, post surgery care, home safety assessments, telemonitoring and much more.

Hospice

Hospice is for patients with life-limited illness. It focuses on comfort goals that are consistent with the patient's choice. The Hospice team of nurses, social workers, aides, chaplains and therapists will work closely with your physician to develop an individualized plan for your care.

Private Home Care

Private Home Care can benefit seniors who need companionship or help around the house, busy families needing assistance or patients needing special care to live safely and comfortably at home.

Home Medical Equipment

Home Medical Equipment helps keep you or your loved ones remain independent and safe at home by offering respiratory services and medical equipment, such as walkers, wheelchairs and hospital beds. Home Medical Equipment also includes Personal Response System, which provides assistance at the push of a button.

Home Infusion Pharmacy

Home Infusion Pharmacy specializes in providing home intravenous therapies, clinical therapy management and other related support services not available from most traditional retail therapies.

Mail Order Program

The Mail Order program is a convenient supply service with home delivery of diabetes products, including blood glucose meters, test trips, lancing devices and lancets. Mail Order also includes supplies for Synagis and Ostomy products.

Personal Response Systems

Personal Response Systems provide an immediate response to any situation 24 hours a day, 7 days a week at the push of a button.

Physician House Calls

A full-time physician, highly trained in Hospice and palliative care medicine, visits Home Health and Hospice patients at home in Central Illinois. The physician works in cooperation with attending physicians when it is difficult for you or your loved one to get to a doctor's office.  For more information on our Physician House Call program, click here.
