The OSF Home Care Services' Diabetes Supply Program is a comprehensive, convenient supply service with home delivery of diabetes products, including blood glucose meters, test strips, lancing devices and lancets, as well as an inventory of well-known and trusted diabetes supply brands.
Service is provided from one central location to meet your needs in a consistent manner.
Products will be shipped within five to seven business days from the 24 hours a day, 7 days a week order center. Convenient phone ordering, availability of 90-day supplies (depending on insurance coverage) and continuity of health care through other services and products offered by OSF Home Care Services are further benefits.
How do I enroll in the Diabetes Supply Program?
The OSF Home Care Services Diabetes Supply Program is open to subscribers of Medicare and many other insurance plans. The initial order must come from your physician.
How do I pay for the Diabetes Supply Program?
Our knowledgeable staff will assist you in understanding insurance coverage criteria, handle all insurance paperwork and file claims on your behalf.
How do I process my order?
Orders can be placed by telephone. To speak to an OSF Home Care Services Diabetes Supply Program representative, please call 888.546.3405 Monday through Friday between 8:30am and 5:00pm. If you call after hours, simply leave a message and your order will be shipped out the next business day. You should receive your order within five to seven business days.
