What We Offer

Admission Services

Anyone can make a referral to OSF Home Care. Referrals and inquiries for care are accepted from family members, patients, hospitals, nursing homes, physician offices and clinics and friends. A doctor's order is usually needed before home care services can begin.

Admission services is available 24 hours a day, 7 days a week. Our experienced staff will identify services available and help coordinate those services to fit referral sources and patient needs. To contact admission services, please call (800) 673-5288.
