Physician House Calls

What We Do

OSF Home Care Services offers the services of a full-time physician trained and highly qualified in Hospice and palliative care medicine to visit OSF Home Health and Hospice patients in Central Illinois. Our physician works in cooperation with attending physicians to provide professional visits in your home when it is difficult for you to get to your doctor's office. This service enables you to be seen by a physician who will attend to your medical needs and answer your questions.

Our physician consults with your physician to help manage your pain and other symptoms, plan your care and keep you safe and comfortable in your own home. They will provide education to you and your family about your illness and care options and, when indicated, discuss Hospice care with you and your loved ones.  They will also see patients for advance care planning or an evaluation for Hospice.

To schedule visits and make patient care as convenient as possible, our physician has specific scheduled days in Bloomington, Galesburg, Peoria, Pontiac and surrounding service areas. To be seen by our physician, please do one of the following:

• Ask your doctor, nursing home staff or a family member to call OSF Home Care Services at 800.673.5288 and schedule a visit for you.
• If you are a patient in a Central Illinois OSF hospital, ask to speak to an OSF Home Care Services coordinator.

The Physician House Calls program coverage is provided by Medicare, Medicaid and many private health insurance plans.
