Bridge Program

Hospice | Bridge Program OSF Home Care Services' Bridge Program is a specialized option for chronically or seriously ill patients who need skilled symptom management, but are not yet ready for hospice or who do not meet the criteria for hospice.

Through care offered by both home health and hospice nurses specially trained in symptom management, attention is directed not only at symptom control and pain management but also at educating you and your family.

The Bridge Program provides the following:

  • Nursing, rehabilitation, aides and social work professionals

  • Individualized care planning which may include counseling and assessment of your special symptom control, pain management and rehabilitative needs

  • Care appropriate for seriously or chronically ill patients receiving aggressive or curative treatment with an emphasis on symptom management and pain control

  • Collaboration with your physician in developing and monitoring goals appropriate for your condition

  • Emotional/counseling support for help coping with the impact of a serious illness

  • Information and coordination regarding community resources

If you and your physician determine that the Bridge Program is the best option for your care, a physician's order will be issued. You must meet eligibility criteria. Guided by you and your physician, our team will develop a personalized plan of care designed specifically for your needs.

The Bridge Program coverage is provided by Medicare, Medicaid and many private health insurance plans.
