90-day supply. Please ask a service representative about details for
your specific insurance plan.

How do I enroll in the Diabetes Supply
Program?
- OSF Home Care’s Diabetes Supply Program is open
to OSF HealthPlans subscribers and Medicare patients.
- The initial order needs to come from your
physician.
- Refill orders may be placed by calling
888-546-3405.
How do I pay for the Diabetes Supply
Program?
- Our knowledgeable staff will assist you in understanding insurance
coverage criteria.
- OSF Home Care will handle all of the insurance paperwork and file
all claims on your behalf.
- Patients are responsible for co-payments and deductibles.
For your convenience, OSF
Home Care accepts MasterCard, Visa and Discover, as well as a personal
check, cash or money order.
