New Hires

Congratulations on your offer of employment at OSF Home Care Services. All offers of employment are contingent on passing your new employee physical. Below you will find a list of Occupational Health locations where you may receive your pre-employment physical.

You will be receiving a letter in the mail presenting you with information on your offered position, as well as a packet of valuable information regarding your position, OSF benefits and other pertinent information you need to be aware of as a new employee.

The packet will also contain a return envelope for you to return the forms you must complete as a new hire. These forms will be included in your new employee packet and are also available on this page.
